To Have Records Sent to Another Physician or Hospital

To request that Doyle Park Family Medicine send your records either to you or to another physician or hospital, click the button on the left. Complete the form and follow the instructions there. Note that we use a copy service, and there is a fee for this service. We must have your signed form and your payment before we can complete your request. Fee required is: $25 to send records to another physician, and $40 if records are copied for your personal use, with an additional $0.25 per page if your records exceed 40 pages (additional page charge payable at the time you pick up your records).
