Electronic Communication Terms of Use Agreement & Disclaimer

Doyle Park Family Medicine (DPFM) is pleased to provide this website as a service to our patients. There are, however, limitations, warnings, and use terms that you must be aware of, and agree to, if you intend to use our website or communicate with us via the Internet:

  • DPFM does NOT render medical advice or practice medicine via e-mail or the Internet. Medical care is highly patient specific and is best rendered through direct contact with one of our physicians. For your safety and privacy, medical problems or conditions should NOT be communicated through the Internet.
  • DPFM agrees to receive communications from you over the Internet for four types of transactions only: mail-order prescription refill requests, non-urgent appointment requests, referral requests, and medical records requests. You agree not to make on-line requests for refills or referrals unless:
    • You have recently discussed the related medical condition with your DPFM physician;
    • You will provide all of the information requested on the form in a clear and understandable form; and
    • Your need for a prescription refill or referral is not urgent or an emergency. Emergencies or urgent care needs should be handled by phone calls to us, or 911, depending upon your assessment of the urgency of your health needs.
  • We do not use a secure or encrypted connection, and therefore caution you against sending any information you consider private or confidential. DPFM cannot guarantee that your intended communication will remain confidential or secure in transit via the Internet.
  • DPFM cannot guarantee that your intended communication to us will be received in a timely manner, if at all. If you have not received a timely response, please call the office.
  • You must be 18 years of age and a current patient of DPFM to communicate electronically with DPFM.
  • You agree to hold DPFM harmless for any and all claims arising from the use of DoyleParkFamilyMedicine.com, or any other electronic communication medium.
  • You further agree to release DPFM from any medical, privacy, confidentiality or other claim or liability arising from your use of the Internet or e-mail.

Doyle Park Family Medicine reserves the right to change information contained on this website at any time, without notice, and does not guarantee the accuracy of the contents herein.
